We have just completed the negotiations with the insurance companies for our plan year which goes into effect August 1, 2019, and we have some updates for the coming year.
PLAN CHANGE HIGHLIGHTS:
- We are continuing with United Healthcare (UHC), and are offering three health plan options: Base Plan, Buy-Up Plan, and High Deductible Health Plan with Health Savings Account (HDHP/HSA). (Please see attached Enrollment Memo for plan details.)
- Our dental coverage is continuing with UHC.
- Our LTD/Life/AD&D coverage is moving to UHC. Your current level of coverage will continue with UHC.
- During this year’s open enrollment period only, employees can enroll in Voluntary Life coverage up to the maximum amount of $150,000 and spouses up to $20,000 without evidence of insurability. This means you do not have to answer health questions to enroll.
- Our Employee Assistance Program (EAP) will move to UHC. Employees can receive 3 face-to-face visits and unlimited phone and web support.
OTHER CHANGES: - UHC has removed the $0 co-pay for primary care office visits for dependents aged 0-18 on the Base plan, but it remains on the Buy-Up plan.
- UHC has removed the $500 per occurrence deductible for inpatient care, but it remains for outpatient care.
- The Base and Buy-Up plans have a slightly higher out-of-pocket maximum and prescription drug copays.
(Note: For your reference, we will be continuously updating the portal with additional 2019/2020 benefits documents in the coming weeks.)

We have just completed the negotiations with the insurance companies for our plan year which goes into effect August 1, 2018, and we have some updates we're excited to share this year.
PLAN CHANGE HIGHLIGHTS:
- We are continuing with United Healthcare, and are offering three health plan options: Base Plan, Buy-Up Plan, and High Deductible Health Plan with Health Savings Account (HDHP/HSA). (Please see attached Enrollment Memo for plan details.)
- Employee premiums for the HDHP/HSA and Buy-Up health plans remained the same this year. The premiums for our Base Plan health coverage are reduced for the 2018/2019 plan year.
- NEW! We’ve now added Enhanced Preventive Prescription Drug Coverage to our HDHP/HSA health plan, where many common prescription drugs are covered 100%. This addition could change the economics of the HDHP/HSA plan versus the Base and Buy-up plans. See the Open Enrollment Memo for more details.
- The deductible for the HDHP/HSA health plan is now embedded at $3,000 per individual family member. So for Employee/Spouse, Employee/Child(ren), or Family coverage, even if you haven't met the full $6,000 deductible, if any one individual has met the $3,000 limit, then that individual has met the deductible for the plan year.
- Primary Care for children aged 0-19 is now covered with a $0 co-pay under the Base and Buy-Up health plans.
- The co-pay for Urgent Care under the Buy-Up health plan has been reduced from $75 to $50.
- We’re switching our dental coverage to United Healthcare. Our new dental plan includes Orthodontia coverage for children under the age of 18 and has increased annual and rollover maximums.
- UHC is honoring existing dental rollover credits from 2017/2018 Guardian plans.
- As we switch our Dental plan to UHC, dental deductible and limits will reset on August 1, 2018; however, they will reset again, with new deductibles and limits, again on January 1, 2019.
- Our Guardian Long-Term Disability (LTD) maximum benefit has increased to $7,500 per month.
SPECIAL NOTE FOR ALL UHC HEALTH PLANS: - United Healthcare has added a $500 per occurrence deductible (in addition to the plan deductible) for inpatient care, as well as for outpatient care if provided outside an ambulatory surgical center or physician’s office.

- Employees who choose the HDHP are eligible to open and maintain a Health Savings Account. We have chosen SelectAccount as our preferred bank for HSAs.
- Our dental coverage will remain with Guardian with a slight change in premiums.
- Our Flexible Spending Account (FSA) plan is unchanged. The maximum limit on the Health FSA remains $1,500 and the Dependent Care FSA remains $5,000.
- Our life insurance coverage and voluntary life coverage remains the same.

They continue to have the same names of "Base Plan" and "Buy-Up Plan", but the benefits are different from last year so please read the attachments that were sent to you via email on July 8, 2015, which include the Open Enrollment Memo.
- Our dental coverage will remain with Guardian with a slight change in premiums.
- Our Flexible Spending Account (FSA) plan is unchanged. The maximum limit on the Health FSA remains $1,500 and the Dependent Care FSA remains $5,000.
- Our life insurance coverage and voluntary life coverage remains the same.

Our medical coverage remains the same: the two-tiered plan with Base and Buy-up options. However, some premiums changed slightly this year.
- Our dental coverage will remain with Guardian with a slight change in premiums.
- Our Flexible Spending Account (FSA) plan is unchanged. The maximum limit on the Medical/Dental/Vision FSA remains $1,500 and the FSA Dependent Care remains $5,000.
- Our life insurance coverage and voluntary life coverage remains the same.

Highlights of the plan changes are as follows: - For this next plan year, we will be renewing our medical insurance with United Healthcare. Our medical coverage remains the same: the two-tiered plan with Base and Buy-up options. However, some premiums changed slightly this year. You will see that the vision portion of the UHC plans is a richer benefit, with routine eye exams every 12 months.
- Our dental coverage will remain with Guardian with a slight change in premiums.
- Our Flexible Spending Account (FSA) plan is unchanged. The maximum limit on the Medical/Dental/Vision FSA remains $1,500 and the FSA Dependent Care remains $5,000.
- Our life insurance coverage and voluntary life coverage remains the same.

the changes to our insurance benefits package. We have just completed the
negotiations with the insurance companies for our 2013- 2014 plan year which goes into
effect August 1, 2013.
Highlights of the plan changes are
as follows:
- For this next plan year, we will be renewing our medical
insurance with United Healthcare and our medical coverage remains the same: the two-tiered plan
with Base and Buy-up options. However, some premiums increased slightly
this year. The premium increases have been shared between the company
and employees.
- Dental coverage has changed and will be provided by Guardian.
- Our Flexible Spending Account (FSA) plan is unchanged.
The maximum limit on the Medical/Dental/Vision FSA remains $1,500.
- Our life insurance coverage and voluntary life coverage
remains the same.
