Highmark Companies is proud to participate in and to match your savings for your retirement through a Safe Harbor 401(k) Plan. Northwest Plan Services (NWPS) is the administrator of our plan.

You may contribute up to 100% of your compensation in pre-tax contributions, not to exceed $19,500 in 2020. Highmark will match 100% of your pre-tax contributions, up to 3% of eligible compensation. If you contribute 5% of your eligible compensation, you will receive the maximum match of 4% of eligible compensation.

The NWPS website provides secure access to your 401(k) savings plan.

401(k) Enrollment Form - For new enrollees to sign up for the 401(k) plan.
401(k) Beneficiary Designation Form - For new enrollees or existing participants to designate or change beneficiaries.
401(k) Deferral Election Change Form
401(k) Rollover Contribution Form - For when a participant chooses to roll money INTO the Plan.
401(k) Death Benefit Request Form and Tax Notification Form - Used by the beneficiary of a deceased Participant.
401(k) Distribution Request and Tax Notification Form - For when a participant is requesting distribution (taxable or rollover to another plan).
401(k) Loan Application Request - To request a loan from the Plan.
401(k) Financial Hardship and Withdrawal Request Form and the Special Tax Notice - For when a participant requests a withdrawal from the Plan due to financial hardship.
Special Tax Notice for 401(k) Plans - Must be reviewed when requesting distributions from the Plan.
